Operating in 10 African countries, m2m empowers women living with HIV to serve as community health workers, providing peer support and access to vital health services. Since its inception in 2001, m2m has employed over 12,000 women living with HIV and reached more than 15 million clients.

This role is part of a USAID-funded initiative focusing on community-based HIV support for children and adolescents living with HIV (CLHIV) aged 0 to 20 in Mpumalanga Province. The programme will be implemented between July 1 and September 15, 2025, across Ehlanzeni, Nkangala, and Gert Sibande districts.

ROLE OVERVIEW

mothers2mothers is hiring nine (9) Linkage Assistants to provide critical support in connecting children, adolescents, and their families with essential healthcare and social services. As a Linkage Assistant, you will work closely with the Department of Health (DOH), Department of Social Development (DSD), local AIDS councils, schools, and other stakeholders to coordinate service referrals and ensure successful follow-through across the continuum of care. You will also mentor community-based teams and maintain accurate linkage records.

KEY RESPONSIBILITIES

1. Partner and Stakeholder Engagement

  • Represent m2m at sub-district and district forums involving government departments, CSOs, and key community stakeholders

  • Build and maintain collaborative relationships with healthcare facilities and community service partners

  • Participate in stakeholder meetings monthly, quarterly, or as needed to support programme goals

2. Coordination of Service Linkages

  • Provide assisted linkage to critical services such as:

    • PrEP for index contacts in discordant relationships and high-risk individuals

    • ART initiation and re-initiation

    • GBV survivor support

    • Child protection, maternal health, and other high-impact services

  • Offer mentorship to Family Mentors and Case Coordinators on a quarterly basis

  • Facilitate or support the formation of Kids Adherence Support Groups for CLHIV aged 10–14 and newly diagnosed children

  • Identify and enrol children/adolescents into the ACHIEVER programme

3. Administration, Reporting & Documentation

  • Maintain accurate weekly linkage records using the designated linkage register

  • Prepare monthly summaries and contribute to quarterly and annual organisational reporting

  • Provide consistent feedback to both internal and external stakeholders on the status of client linkages

  • Participate in staff meetings and collaborate across programme units

QUALIFICATIONS AND EXPERIENCE

  • Qualification as a Nursing Assistant or 3–4 years of experience in a Linkage Officer or similar role

  • Experience in OVC (Orphans and Vulnerable Children) or adolescent health programmes is an advantage

  • Strong understanding of community-based health services and case management frameworks

  • Familiarity with Department of Health protocols and systems

  • Proficient in basic computer use and capable of writing professional reports

  • Fluency in English (written and spoken); local language proficiency is advantageous
